Need to Know
- Competition closes 23.59 on 2 August 2026.
- The prize consists of a £1,000 voucher towards a cosmetic dental treatment plan carried out at Revitalise Dental Centre, St Agnes, Cornwall.
- Entry is open to UK residents aged 18 years or over. Employees of Revitalise Dental Centre and their immediate family members are not eligible to enter.
- Entry is completed by submitting the online competition form and correctly answering the competition question. Only one entry per person is permitted unless otherwise stated.
- The winner will be selected at random from all eligible entries received before the competition closing date.
- The winner will be contacted using the contact details provided on entry via telephone, email and WhatsApp.
- The winner must respond within 7 days of the first contact attempt. If no response is received within this time, the winner declines the prize, is found to be ineligible, or is deemed clinically unsuitable following assessment, Revitalise Dental Centre reserves the right to withdraw the prize and randomly select another eligible winner.
- The voucher is redeemable only against a cosmetic treatment plan, which may include treatments such as:
- Veneers
- Composite bonding
- Tooth whitening
- Other cosmetic treatments deemed clinically appropriate by the treating dentist.
- All treatment recommendations are made solely at the discretion of the treating clinician at Revitalise Dental Centre. Revitalise Dental Centre will always recommend the treatment that is in your best clinical interests and will never undertake treatment that is considered unsafe, inappropriate or potentially damaging to your teeth or gums.
- Before treatment can commence at Revitalise Dental Centre, the winner must attend a Comprehensive Cosmetic Consultation and Oral Health Assessment (£150). This fee is payable separately and is not included within, nor deductible from, the cosmetic treatment plan or voucher value.
- If the winner has recently attended another dental practice, Revitalise Dental Centre kindly ask that you arrange for copies of any relevant dental records and radiographs (X-rays) to be forwarded to Revitalise Dental Centre before the consultation where possible.
- Patients must be dentally stable before cosmetic treatment can commence. Any treatment required to achieve dental stability, including but not limited to fillings, periodontal treatment, root canal treatment or extractions, is not covered by the voucher and must be completed before cosmetic treatment begins.
- The £1,000 voucher will be deducted from the cost of the agreed cosmetic treatment plan only. It cannot be used towards consultation fees, routine examinations, hygiene appointments, emergency treatment, maintenance, repairs or any treatment outside the agreed cosmetic plan.
- If the total value of the cosmetic treatment plan is less than £1,000, no cash alternative, refund or credit will be given for the remaining balance.
- The voucher has no cash value, is non-transferable, non-refundable and cannot be exchanged for cash or any alternative prize.
- Only one promotional voucher may be redeemed per patient and it cannot be used in conjunction with any other promotional offer unless expressly agreed by Revitalise Dental Centre.
- The voucher must be redeemed within 12 months of the winner being notified unless otherwise agreed by Revitalise Dental Centre.
- If the patient decides not to proceed with treatment following the consultation, the consultation fee remains payable and is non-refundable.
- If the patient chooses to discontinue treatment before completion of the agreed cosmetic treatment plan, the £1,000 voucher will be applied on a pro-rata basis according to the proportion of the treatment completed. The voucher will not automatically be applied as the first £1,000 of treatment. Revitalise Dental Centre reserves the right to recalculate the final balance accordingly, and any outstanding fees for treatment already provided remain payable.
- Patients are expected to attend all scheduled appointments. Missed appointments or late cancellations may be subject to the practice’s normal cancellation policy and may affect continuation of treatment under this promotion.
- Revitalise Dental Centre reserves the right to withdraw the voucher or discontinue treatment if a patient repeatedly fails to attend appointments, does not comply with clinical advice or behaves in an abusive, threatening or inappropriate manner towards members of staff.
- Payment may be made by bank transfer (BACS), debit or credit card, or cash.
- In addition to the £1,000 voucher, patients who pay the remaining treatment balance in full before treatment commences will receive a 10% discount where applicable.
- Revitalise Dental Centre offers 0% finance for up to 12 months , subject to status and lender approval. Finance options of up to 5 years are also available, subject to eligibility and the finance provider’s lending criteria.
- Winning the competition does not guarantee treatment. All treatment is subject to a full clinical assessment. The treating clinician reserves the right to decline or modify any proposed treatment where it is not considered clinically appropriate, safe or in the patient’s best interests.
- The voucher is valid only for treatment carried out at Revitalise Dental Centre and cannot be transferred to another individual.
- The promoter accepts no responsibility for entries that are incomplete, lost, delayed or not received due to technical or system failures beyond its reasonable control.
- The practice reserves the right to amend, suspend or withdraw the competition or these Terms & Conditions at any time where reasonably necessary.
- By entering the competition, entrants confirm that they have read, understood and agree to these Terms & Conditions and consent to being contacted by the practice regarding the competition using the contact details they have provided.
